We’re Hiring! – Experienced Case Manager, North East England.

JSP is a leading complex and catastrophic injury case management provider, specialising in acquired brain injury, spinal injury and amputee cases. We provide services across England and Scotland and we have an exciting opportunity for an experienced Case Manager to join our North East team.

The Role

Working Hours: Our standard working pattern is Monday to Friday, 37.5 hours per week. However, we understand that flexibility is important. If you're looking for different working hours, we'd be happy to discuss this with you and will accommodate flexible working wherever possible, taking into account the needs of the service.

Office Base: This is a hybrid role with time spent at our office in Corbridge and travelling to visit clients across a wide geographical area. As regular travel is an essential part of the role, a full UK driving licence is required. A car allowance is provided.

As Case Managers, we work closely with clients and their families to assess rehabilitation needs, commission appropriate services, and lead multi-disciplinary teams, delivering tailored support within the community to help individuals achieve their optimum potential.

Brain injury case management is a highly specialised role that goes beyond coordinating care, it involves comprehensive assessment of physical, cognitive, emotional, and social needs, setting meaningful goals, and ensuring interventions are evidence-based and client-focused. We advocate for our clients, navigate complex health, social care, and legal systems, and continuously monitor progress to adapt plans as needs evolve.

About you:

  • Professionally qualified as a Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, or Psychologist.
  • Must have a minimum of 3 years’ experience within your field of expertise.
  • Previous case management experience
  • Excellent report writing skills.
  • Full UK driving license.
  • Successful candidates will be required to undergo an Enhanced DBS

We also want to ensure that individuals with disabilities are provided reasonable facilities to participate in job applications or interviews to perform essential job functions. To support this, we are a Disability Confident employer.

At Active Care Group we want everyone to feel welcome and supported. We encourage applications from people of all backgrounds and experiences. If you need any reasonable adjustments during the recruitment or interview process, just let us know and we’d be very happy to discuss this with you.
