Brownbill Case Management has been established since 1992 and exists to deliver an outstanding case management service to children, young people and adults who have enduring and complex health conditions and injuries.
As part of the Active Care Group (ACG) Case Management Division, Brownbill Case Management Services (BAL) provides a specialist case management service primarily for individuals who have been affected by brain injury, spinal cord injury, Cerebral Palsy, complex orthopaedic injuries and amputation. We work across the Midlands, London and Home Counties, and South East regions
We are currently interested in Associate Case Managers to support our specialist case management services, including assisting with arranging complex packages of care, coordinating rehabilitation and healthcare, procuring specialist equipment, and facilitating educational, vocational and leisure opportunities.
For this role you will need to be a driver with ability to travel and hold a UK professional health qualification which requires you to be HCPC, NMC or Social Worker Registered. It would also be an advantage if you are an Advanced Members or working towards advanced membership status of the British Association of Brain Injury Case Managers (BABICM).

What you'll be working:
- We can consider both full-time and part-time (minimum of 3 days p/wk preferred)
- Our coverage is London & Home Counties, Midlands, Kent, Essex and the South Coast, candidates from these areas would be highly desirable

What you'll be doing:
- Advocating for the Client’s needs (both adults and children) while building strong working relationships with families, deputies and litigation solicitors
- Produce Initial Need Assessments, action plans and SMART goals with timeframes that are specific and relevant to the client
- Recruit, select and manage a team of care/support workers for Clients
- Implement an individual client focused Support & Rehabilitation Plan with specific guidelines for the support team
- Compile and manage MDT team of required professionals to deliver the rehabilitation plan
- Monitor and review the Case Management Plan and Support & Rehabilitation Plan, amending as per the Client’s needs
- Compile risk assessments on key activities as required and identify risk management strategies that will manage the safety of all parties in a positive manner
What you'll have:
- At least 3 years experience within a Case Management role
- Hold a UK professional health qualification which requires you to be HCPC, NMC or Social Worker Registered
- Full Driving Licence and access to a car
- Experience supporting people with neurological conditions in a community setting - this could be adults or children
- Thorough knowledge of the CQC fundamental standards and key lines of enquiry
- Full working knowledge of the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS) and Safeguarding frameworks
- Ability to address and respond to employee issues in a fair and consistent manner
- Strong organisational, time management and planning skills
- Flexible, professional and positive approach to multi-site working
